Thing is it could be a number of things.
The worst case scenario is that either your CPU or GPU are heating up and in order to cool it down, the fan kicks up a storm.
But saying that, if it were an overheating issue, then as soon as the Macbook would warm up, it would shut itself down to stop any thermal damage to itself. If it is not shutting down, then it could be as simple as that you are runninga lot of processes in the background.
If that is the case, then the more processes you run in the bacground, then the harder the machine has to work in order to fulfill those processes, hence why it heats up more. Its like the same principle of any machine, the harder you push it, the hotter it gets.
If you really are worried, then take it into a local Mac repair centre, they can tell you if it is thermal issues or just general usage heat.
If it is thermal issues, and its either your CPU, GPU or the Logicboard then you may as well buy a new Macbook.
